AKC FAST CAT Refund Request

Use this form to request a refund for entry fees paid for an upcoming FAST CAT Trial. There is a $5.00 processing fee per entry for this service. As a part of this service your dog will be removed from the run list, time sheets, and results reports. Requests for refunds will not be accepted within 48 hours of the trials.

If you have more than one dog entered you must complete a form for each dog for each Event/Trial.
